Professor Eliane Gluckman is one of the co-chairs of the upcoming ESH Updates in Clinical Hematology conference to be held in Paris on February 9-10, 2012 in Paris.

This conference is part of a new series features: a 1,5 day update on patient management from world class experts, a clinically and practically oriented program that reflects most recent state-of-the-art as well as innovation, in a mixture of different hematologic disorders, – in-depth presentations followed by interactive discussions, including clinical cases and controversies, and a handout comprising lecture summaries, slides and recent, selected references.

Eurocord-Ed Policy Document on Cord Blood Banking in Europe

Since the first cord blood transplantation was performed in Europe in 1988, a large number of cord blood banks have been established in order to facilitate unrelated cord blood transplantation. Over the years the number of units collected in Europe has increased to approximately 190 000 in 2011. Cord blood transplantation is a rapidly growing field with an increasing number of indications worldwide. The results reported by Eurocord reflect a strikingly rapid increase in activity. For example, the Asian Pacific Blood and Marrow Transplantation Group reported 937 cord blood transplants in 2008 alone. In the USA, more than 40% of paediatric unrelated transplants are provided by cord blood.

Highlights from the 2011 World Cord Blood Congress

The ESH, Eurocord, Eurocord-Ed EBMT World Cord Blood Congress held in Rome, Italy from October 27-29, 2011 was a tremendous success thanks to the beautiful surroundings offered by the historic site in the city centre, the quality of the scientific programme, and the lively interaction with attendees. 
The congress took place in the centre of Rome on the Piazza Montecitorio adjacent to the Italian parliament building. Over 320 participants from fifty-two countries and from as far away as Canada, Argentina China and Australia attended the event.
The 2011 meeting focussed on Cord Blood Transplant, immunological aspects, and clinical results. All sessions were divided into three key sections: the current State-of-the-art, expert short communications, and  panel discussions.
According to attendee feedback, the possibility to discuss openly with internationally renowned specialists in both scientific and social settings was particularly appreciated   The local organizers: William Arcese, Franco Locatelli, and Paolo Rebulla all deserve special thanks as do the Staff of the European School of Haematology. Revision of the Clinical Trials Directive 2001/20/EC Concept Paper for Public Consultation

The European Commission is planning to put forward, in 2012, a legislative proposal to revise the Clinical Trials Directive 2001/20/EC.
This concept paper [94 KB] is being put out for public consultation. It presents:

• a 'preliminary appraisal' of which option appears to be the most suitable one to address some of the key concerns of the Clinical Trials Directive, on the basis of the current state of the impact assessment; and
• the main figures that are being used to evaluate the impacts of the different policy options.

European Medicines Agency releases new guidance on medicines based on SCs

The European Medicines Agency has released new guidance on medicines based on stem cells. The guidance covers the different types of stem cells used in medicines, and the considerations that companies need to take into account when developing medicines including stem cells or developed from stem cells.
The reflection paper on stem cell-based medicinal products (available here)  stresses the fact that companies developing medicines including stem cells need to pay close attention to the way the medicines are manufactured, to make sure that the final medicine is as consistent and reproducible as possible.
Pre-clinical and clinical testing also needs to take account of the cells' properties, ensuring that the possible risks of tumour development and rejection by the body are studied adequately and balanced against their benefits for patients.
Stem cells are naturally occurring cells in the body that have the ability to divide and produce a range of different cell types. The use of stem cells as medicines is a promising area of research as they may be able to help the body to regenerate damaged or lost tissue in diseases like Parkinson’s disease, multiple sclerosis, heart disease, liver disease, spinal cord damage or cancer.
The reflection paper was adopted by the Agency's Committee for Advanced Therapies (available here ) (CAT) on 14 January 2011. This followed a public consultation between March and June 2010 (see the overview of comments  here ) and discussion at a public workshop in May 2010 here involving 230 participants from academia, industry and regulatory authorities. Testing Power of Cord Blood

Researchers at Monash Medical Centre in Melbourne, Australia, are  awaiting approval for a study that would use umbilical-cord blood to  treat 20 children with cerebral palsy. The researchers want to evaluate

The World Health Organisation presents their guidelines for transplantation of human cells, tissues and organs. According to the WHO "In 50 years, transplantation has become a successful worldwide practice. However, there are large differences between countries in access to suitable transplantation and in the level of safety, quality, efficacy of donation and transplantation of human cells, tissues and organs. The ethical aspects of transplantation are at the forefront. In particular, the unmet patients’ needs and the shortage of transplants lead to the temptation of trafficking in human body components for transplantation." Eurocord is an international registry of cord blood transplants established in Paris, in 1995 under the direction of Dr Vanderson Rocha and Professor Eliane Gluckman. It has currently collected and validated more than 6000 cases of cord blood transplants throughout the world with approximately 70% provided by EBMT transplant centers.

EUROCORD collaborates closely with cord blood banks and EBMT centres, collecting and validating clinical data of patients transplanted with Netcord cord blood units, or receiving a cord blood transplant in an EBMT centre. This collaboration is important in order to evaluate the quality of the cord blood unit and to help cord blood banks to follow national and international system accreditation.

EUROCORD also collaborate with EBMT working parties in observational studies. In 2009, the EBMT Registry has finalized a data agreement with the EUROCORD Registry, to improve data collection in Promise. The agreement facilitates the data flow, enabling EBMT centres to submit their cord blood data once, instead of to both organizations.

In December 2009, in order to collect data of American patients receiving a European cord blood graft and vice-versa, an agreement between Eurocord-EBMT and CIBMTR was established to share clinical data outcome through the implementation of AGNIS project. AGNIS is "A Growable Network Information System", which has the goal to transmit haematopoietic stem cell transplant (HCT) data between different data bases. This project was approved and financed by the American government and it is in currently development between Eurocord and CIBMTR.

In 2009, Eurocord registry has published many articles (see below) in collaboration with EBMT working parties and National Societies. Two abstracts were accepted in the EBMT plenary session in Goteborg 2009. Prospective trials in collaboration with The French Society of HSCT (SFGM-TC) have started. Many other observational studies are on going. The main task of the Eurocord registry in 2010 is to finalize the AGNIS project and to continue and establish collaborations with cord blood banks and EBMT working parties. Many observational studies are on going and we hope to finalize most of them.

Umbilical cord blood is now considered a drug by the FDA.

The new Guidance for Industry: Minimally Manipulated, Unrelated Allogeneic Placental/Umbilical Cord Blood Intended for Hematopoietic Reconstitution for Specific Indications requires cord blood banks to measure identity, potency (strength) and quality of cord blood prior to its intended use. The parameters recommended by FDA/CBER are often at odds with, and in many cases, apparently contradictory to, the regulations. The Guidance allows for an "alternative approach" and other comparable and equivalent assays to be performed, providing the assays have been validated.

This webinar discusses the recommended assays in the Guidance and alternative, validated assays that have been designed specifically for the intended purpose.

Article 10, paragraph 2 of the European Directive 2004/23/EC requires the Competent Authority to establish and maintain a publicly accessible register of tissue establishments specifying the activities for which they have been authorised. In addition, paragraph 3 states "Member States and the Commission shall establish a network linking the national tissue establishment registers".

The Eurocet Project, which was funded by the European Commission, has established The Eurocet Registry which will include the name and address of the Tissue Establishment, and the type of tissue and / or cell to which the authorisation pertains. The Ospedale Maggiore hospital in Milan was originally founded in 1456. The "Policlinico" (General Hospital), Mangiagalli and Regina Elena Ospedale Maggiore Foundation was officially opened in 2005. The mission of exploiting the potentials of cellular therapy and of performing biobanking research was given to a unit named the Center of Transfusion Medicine, Cellular Therapy and Cryobiology (CTMC).

Currently, the CTMC includes five sections: one of which includes the Milano Cord Blood Bank, with an inventory of 6627 allogeneic cord blood units, 350 of which have been transplanted in 21 countries (39 units in 2007). Published March 6, 2009

Advanced Therapy Medicinal Products (ATMP) Regulations 1394/2007/EC 20th February, 2009

An ATMP is a medicinal product as defined in Directive 2001/83/EC and is either:

• a gene therapy medicinal product as defined in Part IV of Annex I to Directive 2001/83/EC;
• a somatic cell therapy medicinal product as defined in Part IV of Annex I to Directive 2001/83/EC; or
• a tissue engineered product as defined in Article 2 1 (b) of the ATMP Regulation (1394/2007/EC).

Since the first human cord blood transplant, performed twenty years ago, cord blood banks have been established worldwide for collection and cryopreservation of cord blood for allogeneic hematopoietic stem cell transplant. A global network of cord blood banks and transplant centers has been established for a common inventory and study of clinical outcomes.
